Description: All scholarship recipients will receive a monthly grant of $537 regardless of whether they attend an Illinois community college or university. The grant is effective the first day of attendance and will terminate at the end of the academic year. This includes recipients who attend summer school and maintain full-time status as defined by the college/university.
Health Documentation Due to JTHS by October 15

Dear JTHS families,
The deadline to submit health documentation required by the Illinois School Code is Thursday, October 15, 2020. This documentation is required for your student to continue attending school.
Ninth grade and transfer students must submit the completed State of Illinois Certificate of Child Health Examination Form.*** The form is a two-sided document and both sides must be filled out. On page 2, there is a section called “student health history” which must be filled out by the parent/guardian and MUST BE SIGNED and dated. The physical is considered incomplete without this section. In addition, please make certain that your student’s healthcare provider has documented the height, weight, blood pressure and BMI on the form and has completed the diabetes screening.
All seniors must submit proof of receiving a meningitis shot on or after their 16th birthday.
