Dear JTHS Families, 

We are contacting you regarding important vaccination requirements for your senior student.  Twelfth grade students enrolled at Joliet West must provide documentation of meningococcal conjugate vaccination (MCV), including the date of administration and a doctor’s signature. 
 
A meningococcal conjugate vaccination (MCV) is required on or after 16 years of age regardless of past vaccine dates.  Proof of at least one dose of a recent meningococcal conjugate vaccination (MCV) that has been administered on or after 16 years of age is required of all students entering the 12th grade.  

Students who do not provide proof of vaccination, will not be allowed to attend school in August.
