The Health Examination Form with proof of immunizations is due prior to the first day of school.
If this document is not submitted by October 15th, students will be excluded from school. This exclusion shall begin immediately and shall remain in effect until the student is in compliance according to the Illinois School Code (105 ILCS 5/27-8.1).
All incoming freshmen students are required to submit:

The two page document must be filled out completely. On page 1, there is a section called “student health history” which must be filled out, signed, and dated by the parent/guardian. The form is considered incomplete without this section. Please make certain that your student’s healthcare provider fills out the form to its entirety. The form has a series of questions that must be answered by the provider conducting the physical exam. In addition, students entering grades 7 through 12 who have not already received a Tdap vaccination are required to receive 1 Tdap dose regardless of the interval since the last DTaP, DT or Td dose. Failure to complete any of the above will result in the form being returned for completion.
