Class of 2022 Senior Vaccination Requirement

All students entering their senior year students must provide documentation of vaccination, including the date of administration and a doctor’s signature.  The (MCV) vaccine is required on or after 16 years of age regardless of past vaccine dates.  Proof of at least one dose of a recent (on or after 16 years of age) meningococcal conjugate vaccination (MCV) is required.

Central students should submit vaccine documentation electronically to acarroll@jths.org. West students should submit vaccine documentation electronically to mfuentes@jths.org.  Documentation can also be dropped off at the Central or West Main Office Monday – Thursday from 7:15 a.m. to 4:30 p.m.
