JTHS Families, 

VNA Health Care and JTHS have partnered once again to offer dedicated appointment times for JTHS students who need a school physical, sports physical, and/or any required immunizations for the upcoming school year. 

Important Scheduling Information:

  • These appointments are reserved exclusively for JTHS students, including incoming freshmen from the class of 2030, and are not available through VNA Health Care’s general call center or clinic staff. 

  • Appointment slots are limited, so early scheduling is strongly encouraged! 

  • To schedule an appointment, please call 630-892-4355 and press 8520 to speak to a dedicated scheduler. Be sure to mention that you are scheduling an appointment for a District 204 student. 

Appointment Dates and Locations for JTHS Students  

  • When: July, August, September, and October 

  • Cost: $25 for school physicals, sports physicals and/or immunizations. The cost may be lower depending on your family’s income and insurance coverage. Immunizations and laboratory services will be billed separately. 

VNA accepts Medicaid, Medicare, and both insured and unsured individuals. They also offer a sliding fee scale based on household income. Staff are available to assist families wishing to apply for benefits. View the VNA Health Care Appointment Flyer (English/Spanish) for details.    
  
A parent or guardian must be present at the time of the appointment.  A copy of the child’s vaccination record is required at the time of the visit. To help expedite the registration process, families may complete the Student Registration and Consent Form before their appointment and bring it with them to the visit:

Per the Illinois School Code, please review the following health requirements: 

Health Requirements for Freshmen  
Families of incoming freshmen are reminded that Illinois State law requires students entering 9th grade to submit a completed State of Illinois Certificate of Child Health Examination Form, including proof of immunizations. The physical examination must be completed within one year prior to entering high school and must be signed by a licensed healthcare provider. Parents or guardians must also complete and sign the student’s health history section of the form. Pursuant to Illinois School Code (105 ILCS 5/27-8.1), students must submit the completed Child Health Examination Form with immunizations by October 15, 2026, or face exclusion from school beginning on that date. Additionally, all incoming freshmen students are required to submit a completed Proof of School Dental Examination Form by May 15, 2027.

Health Requirements for Senior Students 
Students who will be seniors during the 2026-2027 school year are required by Illinois State law to provide documentation of one dose of the meningococcal conjugate vaccine administered on or after their 16th birthday. Pursuant to Illinois School Code (105 ILCS 5/27-8.1), students must submit proof of immunization by October 15, 2026, or face exclusion from school beginning on that date. 

Deadline to submit health documentation: October 15, 2026. Failure to comply will result in exclusion from school starting on October 15.
