Freshman Physical

The state of Illinois requires every incoming freshman to have a new physical.

The Illinois school code requires each student to have a medical examination and a complete immunization record from infancy, before attending the ninth grade at Joliet Township High Schools.  We are not allowed to admit any student into school until the examinations required by law have been made and satisfactory evidence is presented to and accepted by the staff members of our Health Services Department.  Students will not receive their schedule in August, and may be late starting school, unless Joliet Central High School receives a valid Health Form.

NO APPOINTMENT CARDS WILL BE ACCEPTED IN PLACE OF A PHYSICAL!

We urge you to arrange for this physical examination as early as possible to avoid the rush on physicians during the last days before the new school term.  By doing so, you will avoid the disappointment of your child being ineligible to begin school on time because this requirement of law has not been met.  If you do not have a record of your child's immunizations please contact the Junior High immediately to obtain a copy.

Below is a link to the most current health form provided by the State of Illinois.  Please print the form and take it to your child's doctor to be completed.  All sections of the form must be completed as well as appropriate signatures.  A parent/guardian MUST complete and sign the back of the form.
